Discussion:
This test can be used in a variety of ways.  It can be used to test readiness for entry to or exit from a program.  It can be used to test interpreting ability. It can be used to assess reading skills.

00. Sample question:
A) Mom wants to know "Where is the restroom?"
B) Dad wants to know "Where is the restaurant?"
C) Mom wants to know "What do you think about the new restaurant?"
D) Talking is not allowed in the library.
E) Mom is a lawyer.

Discussion:
For example the student can be provided a copy of the test and the instructor can sign one of the sentences in ASL whereupon the student would select the answer that most closely corresponds to the sentence signed.
Note: The ASL glosses (written form) or sign choices may differ considerably from the English version and yet be accurate.  For example, the phrase, "wants to know" could be signed "curious" in sentence C)  Mom wants to know "What do you think about the new restaurant?
Does it have to be signed "curious?" No.  But when taken as a whole, the phrase likely indicates that "mom" is curious as to your "opinion" of the restaurant.  So if you signed, "MOM CURIOUS YOUR OPINION RESTAURANT" I'd certainly consider your signing to be correct because it is obvious to me that you have a strong command of both languages.
The choice between topicalizing the sentnece, "RESTAURANT? MOM CURIOUS YOUR OPINION" and using the subject verb object version of the sentence is immaterial here because in pragmatic terms I don't know whether you have estalbished the topic of the restaurant in your conversation yet or not.  If you have already introduced the topic during your conversation and it is obvious that the listener doesn't need it reintroduced then there is no reason to topicalize that sentence.  Such pragmatic considerations are better tested in an interactive discourse-based (conversational) test.
